Donna Dannar 8/9/06 Regarding Length of Time between Consent for Evaluation & Determination of Eligibility... According to my memory and the minutes from the 2/24/06 training, this new procedure does not apply to children who were previously on IFSP transitioning to an IEP, yet they are on the data verification report. Also according to Toni Van Cleve, we are not expected to retrieve this information for initials that occurred prior to staff being trained (our staff were trained 2/6/06), yet those records are also on the data verification report. So are we supposed to go back and research these items to make the data verification report complete?
     
Toni Van Cleve 12/11/06 This issue needs to be clarified. Even when a child has been on an IFSP, if he or she is being evaluated for Part B, there must be a Consent for Evaluation signed by the parent. A child must to be determined eligible for Part B services. The statute does not allow an automatic transfer for children who have been on an IFSP. It is correct that the state is not requiring entry of data received prior to the training.
