| Toni Van Cleve | 8/12/08 | You are correct, the Goal Codes will no longer be collected. For now you will just need to leave that field blank. There is a new Medicaid Consultant at the DE, Jim Donoghue. He will be reviewing the form and making any needed corrections. |
