| Dee Gethmann | 9/11/08 | A change in Service Coordinator is a Change Reason of CRI - change roster information and is not a Meeting Type. IMS data entry personnel document this information on the “Service Information” Screen, in the field labeled, “Federal Exit & Roster Change.” This information is found on the IFSP Template, “IFSP Early Intervention Services”, in the section labeled “Discontinue/Change Reason”. The Bureau will be reviewing the Meeting Types table to align the table with the IFSP forms. |
