IEP forms for attending out-of-state
Patti Brown | 9/17/10 | We have a student who is attending a behavior program out of state. Our question is this. Will this student remain on Iowa IEP forms or do they need to do the review on Illinois forms. There is some disagreement here on what needs to happen. There is currently no way for the out of state program to access Iowa IEP program, so they are proposing the resident district does all IEP paperwork. |

The Illinois district/program needs to do the IEP on their own forms, since the student will be receiving services in Illinois. In an out of state placement, it is the responsibility of the Iowa resident district to adhere the following procedures: -Ongoing follow-up of the student's IEP progress, which includes progress monitoring reports that will be documented and maintained in the student's LEA file -Participation in IEP meetings via telephone conferencing, video conferencing, or physical presence by LEA staff and/or AEA staff. Resident district representatives must be invited to all IEP meetings -Documentation of an annual IEP review and copies of the current IEP maintained in the resident district file. |
