Signed Copy of Parent Permission for Medicaid

Mary Jo Nordheim 04/15/2013 When we get an IEP in from the school and the child has Medicaid services and has a Medicaid #, if we do not get a signed copy of the parent permission form in with the IEP, how important is it that we track down the signed form to enter the date in IMS? I understand for our AEA services, we would have the form, but what if it was just an LEA billable service, i.e. transportation.

The agency doing the billing (LEA, AEA or both) needs a copy of consent.  The documentation in the Web IEP of signature date does not suffice for documentation of consent.
